Drug & Alcohol Rehab Admissions

Praxis provides a simple admissions process available daily from 11 a.m. to 7 p.m. to get you or your loved one into treatment. Here is an easy guide to follow below:

  1. Prior to admission into Praxis, you will speak with an admissions team member at 833-772-9471
  2. Our admissions team will confidentially gather all the necessary information to arrive at the next best course of action to take in the treatment process.
  3. Our admissions team will gather the necessary insurance information and then walk you through the payment options prior to admission.
  4. Upon admission, a thorough history and physical examination will be performed by our medical staff to ensure a safe and secure transition into our program.
  5. Prior to admission, we ask that any knowledge which might be helpful to your situation be made available. This information typically includes previous treatment experiences or other conditions which would help assist our treatment of you.
  6. The appropriate releases of information to other professionals will be made available by Praxis.
  7. Some of the exclusionary criteria for admission into the Praxis program include cases involving acute psychiatric conditions. In these instances, a higher level of care is more appropriate.

Patient Packing List

  • Photo ID: Driver’s License or Passport
  • Medical Insurance Cards
  • Medication list including dosages and all know allergies to medications
  • Comfortable clothing and shoes. (Pack amount used for a week)
  • Athletic shoes and clothing for fitness classes (Yoga)
  • Alcohol free toiletries, including shampoo, mouthwash, soap and deodorant
  • Toothbrush, toothpaste, shaving items and hairdryer
  • Writing materials for taking notes
  • Premade List of contacts including names and phone numbers of loved ones you would like to contact while in treatment.
  • Alarm Clock (without radio)
  • Stamps and Envelopes
  • Prepaid gift cards to order snacks online while in treatment.
  • Prepackaged snacks in factory sealed containers
  • Drinks that are factory sealed (no energy type drinks)
  • Family photos
  • Books
  • Spiritual or religious items
  • MP3 player (no camera or connection to Wi-Fi)
  • Do not bring
  • Cash, Checkbook, credit or debit cards
  • Drugs or Alcohol or Narcotics of any kind
  • Aerosols
  • Over the counter medication
  • Heating pads or electric blankets
  • Hemp products
  • Magic markers, sharpies, white out
  • Marital aids, condoms
  • Cleaning supplies
  • Perfume, cologne, aftershave, nail polish, polish remover
  • Knives, scissors, sharp objects and other items that could be considered weapons
  • Cellphones, laptops, tablets, cameras and mp3 players (with cameras) Video games or DVD
  • Pornography
  • Revealing clothing (tank tops, sleeveless shirts, midriff baring tops)
  • Candles and incense
  • Clothing with profanity or references to drugs, sex or alcohol

Admissions Requirements
All patients admitted to Praxis will complete a screening and assessment process conducted face to face by an Intake Clinician and Intake Nurse. Praxis utilizes the ASAM Criteria to determine a patient’s appropriateness for admission and to determine level of care and length of stay required to meet patient needs.

Patients who meet one or more of the following conditions shall not be admitted to Praxis for any level of care; if existing Praxis patient begins to exhibit the below conditions, s/he shall be referred expeditiously to an appropriate outside facility:

Non-ambulatory

  • Needs assistance with any activities of daily living (bathing, dressing, grooming, etc.).
  • Incontinence.
  • The patient is actively suicidal, as noted by actual attempts at suicide or if patient endorses detailed, premeditated plan coupled with intent and available means.
  • Patient demonstrates violent behavior (including verbal or physical aggression) or endorses homicidal ideation towards staff or other patients or has detailed, premeditated plans for such behavior.
  • Patient has either diagnosed or undiagnosed mental health disorder that presents with such severity that it will prevent the patient from actively participating in his or her treatment in a residential setting.
  • Patient has an acute, life-threatening illness or condition that requires transfer to an inpatient level of care.

Patients who are less than 20 weeks pregnant may admit to any level of care other than detoxification. Patients who are 20 weeks pregnant or more may not admit to detoxification or residential treatment.
